Forgetfulness and memory problems can be a normal process of aging but it can also be a sign of something else.  If you or a loved one notices subtle memory changes do not automatically assume it is “dementia” because it may be related to other factors.

Touchpoints at Bloomfield entered into a partnership designed to bring continuity in pulmonary care from the acute hospital setting into the post-acute, skilled nursing and rehab care setting. The program will address repeat hospitalizations by applying integrated pulmonary care and sleep medicine consulting to many diagnoses, including COPD and congestive heart failure.
