Touchpoints Rehab at Bloomfield and Manchester are proud members of the Saint Francis Hospital - Trinity Health New Englend Post-Acute Care Network (PACN). As such we admit and care for patients in the network, including and especially patients with Congestive Heart Failure (CHF). Saint Francis Hospital, in conjunction with its Hoffman Heart and Vascular Institute is holding an educational series, taught by esteemed members of their staff, for patients with CHF and their families.
